Living with a chronic condition like heart disease means that what you do every day matters just as much as what happens at your next appointment. The choices you make between visits — what you eat, how you move, how you manage stress, how consistently you take your medications — all of it adds up. But no one develops healthy habits overnight. And no one should be expected to overhaul their entire life without guidance, support, and a plan that actually fits their real world.
That's where I come in...
As a Community Health Worker and Patient Advocate, I work alongside patients to build sustainable, realistic habits that support their health and slow the progression of their condition. I provide education that meets you where you are, tools you can actually use, and ongoing support that keeps you moving forward even when you feel as if you can't. Having someone to help you make sense of it all makes the difference between managing your condition — and letting your condition manage you.
